Viral Croup

🃏 Order set for Viral Hepatitis

• Croup (Laryngotracheobronchitis) — viral (Parainfluenza virus type 1 most common), age 6 months–3 years; barking/seal-like cough, stridor, hoarse voice, low-grade fever
• Westley Croup Score (0–17):
  • 0–2 = Mild; 3–5 = Moderate; 6–11 = Severe; ≥12 = Impending respiratory failure
  • Items: stridor (0/1/2), retractions (0/1/2/3), air entry (0/1/2), cyanosis (0/4), consciousness (0/5)
• Differential: epiglottitis (toxic, drooling, tripod, no barking cough, thumbprint sign), bacterial tracheitis (high fever, toxic, not respond to dexamethasone), foreign body, peritonsillar abscess, angioedema
• Red flags (epiglottitis): drooling, tripod position, muffled hot-potato voice, ไม่ให้แลุชมโดยเด็ดขาดแต่ controlled environment
• Croup เกิด mainly ปั่น — worse at night, cry = worse (agitation increases obstruction)

🔬 Investigation

• Clinical diagnosis — ไม่ต้อง X-ray routine; CXR ถ้าสงสัย foreign body หรือ atypical
• Neck X-ray lateral ถ้าสงสัย epiglottitis (thumbprint sign) — only if stable + controlled; อย่าให้นอน
• SpO₂ monitoring — ถ้า moderate/severe
• CXR: steeple sign (subglottic narrowing = croup) ถ้าสงสัย

💊 Medication

• Dexamethasone PO/IM/IV:
  • 0.15 mg/kg (mild) — dose เดียว (OPD sufficient ถ้า mild)
  • 0.6 mg/kg (moderate-severe, max 10 mg) ครั้งเดียว; สามารถ PO ได้ถ้า tolerate; ไม่จำเป็น repeat เพราะ effect ยาว >24 ชม. (Cochrane 2018)
• [Moderate-Severe — Nebulized Racemic Epinephrine / L-Epinephrine:]
  • L-Epinephrine 1:1000 solution: 5 mL (5 mg) nebulize โดยไม่ต้อง dilute
  • Onset 10–30 min; duration 2 ชม.; ต้อง observe 2–4 ชม.หลัง (rebound risk)
• O₂ ถ้า SpO₂ <94%; HFNC ถ้า respiratory distress
• คา calm เด็ก + ให้อยู่กับผู้ปกครอง — agitation เพิ่ม obstruction; หลีกเลี่ยงอุปกรณ์ที่ทำให้เด็กเครียด
• Heliox (70:30 He:O₂) ถ้า severe + intubation not yet decided

• Discharge criteria (mild-moderate with good response):
  • ไม่มี stridor at rest, SpO₂ ≥94% room air, tolerating oral, ไม่ distress (หลัง observe 2–4 ชม. post-epinephrine)
• Admit indications: moderate–severe (Westley ≦6), stridor at rest, SpO₂ <94%, age <6 months, poor social situation
• Parental education: viral, self-limiting 3–5 days, ให้ cool mist/humidifier (evidence weak but accepted), return precautions
