Unknown Animal Bite

• Rabies risk assessment: สัตว์อะไร (dog, cat, bat, fox = high risk; rabbit, rodent = low risk), geographic area (endemic or not), vaccination statusของสัตว์
• WHO exposure category:
  • Cat. I: touching/feeding, no break in skin → wash only
  • Cat. II: nibbling, minor scratch → wash + vaccine
  • Cat. III: single/multiple transdermal bites, contamination of mucous membrane/wound → wash + vaccine + RIG
• Animal status: สัตว์มีอาการผิดปกติหรือเปล่า? สัตว์จรหนีหรือไม่พบ? ถ้า healthy domestic และ observe ได้ 10 วัน → ถ้าไม่เป็นโรคปกติ = PEP continue
• Wound type: puncture, laceration, crush, location (face/hands/neck = high risk)
• Immunocompromised: steroids, HIV, transplant → Cat. II ก็ให้ RIG
• Pre-exposure vaccination (PrEP): ถ้าได้ PrEP (3 doses) = post-exp ให้แค่ D0+3 อิก 2 doses เท่านั้น (ไม่ต้อง RIG)

🔬 Investigation

• ไม่มี lab จำเพาะสำหรับ rabies — diagnosis ทาง clinical/epidemiological
• CBC ถ้าสงสัย wound infection
• X-ray extremity ถ้าสงสัย fracture หรือ embedded tooth/foreign body

💊 Medication

• [Wound first aid (immediate, most important):]
  • ล้างแผลด้วย soap + running water อย่างน้อย 15 นาที
  • Irrigate ด้วย NSS/povidone-iodine (10% diluted)
  • ห้ามเย็บแผล primary suture (wound bite มี rabies risk สูง) — delayed closure (3–5 วัน) หรือ open wound ถ้าทำได้
• [Cat. II/III — Rabies PEP:]
  • PVRV (Purified Vero Cell Vaccine) ตาม Zagreb or Essen regimen:
    • Essen: D0, D3, D7, D14, D28 (5 doses)
    • Zagreb: D0x2, D7, D21 (4 doses)
  • [Cat. III + ไม่เคย vaccinated] — Rabies Immunoglobulin (RIG):
    • HRIG 20 IU/kg หรือ ERIG 40 IU/kg: ฉีด เข้าในและรอบแผลทุกจุด (infiltrate + remainder IM distant)
    • ให้พร้อมกันกับ D0 vaccine (คนละตำแหน่ง)
• Tetanus toxoid (TT): ถ้าไม่ได้ booster ใน 5 ปี; + TIG ถ้า unvaccinated + dirty wound
• ATB (animal bite): Amoxicillin-Clavulanate 625 mg q8h x 5–7 วัน — ถ้าบาดแผลลึก, ดุสยูกเสี่ยง (cat bite นิยมติด Pasteurella), hand/face bite, immunocompromised

🔬 Investigation

• เฝ้าติดตามแผล q24–48h — cellulitis, lymphangitis signs
• ติดตามสัตว์ 10 วัน ถ้าสัตว์ยังมีชีวิตอยู่; ถ้าตายภายใน 10 วัน = rabies confirmed → continue PEP
