Tropical Infectious Diseases (Malaria, Rickettsia & Vector-borne)

🎯 Board-exam takeaway: ไข้เขตร้อน = approach จาก acute undifferentiated fever → แยก severe malaria ออกก่อน. Severe malaria (any species) → IV artesunate (push, half-life 20 นาที); quinine ต้อง drip ห้าม push (QT → ตาย). Non-severe falciparum จากศรีสะเกษ/อุบล → artesunate-pyronaridine (DHA-PIP resistant). Rickettsia (scrub typhus = eschar + severe headache) → doxycycline.
🧬 Etiology & Framework
Tropical infection = โรคเขตร้อน (ใกล้เส้นศูนย์สูตร: SE Asia, Africa, India, S America). ส่วนใหญ่มาด้วย acute undifferentiated fever → แยก self-limited vs life-threatening; แยก malaria ออกกลุ่มหนึ่ง แล้วจึงไป viral/bacterial/parasite.
🩺 Malaria — Clinical Phenotypes
Plasmodium 5 species: falciparum (severe เด่นสุด), vivax, ovale, malariae, knowlesi. Severe malaria = 3 species: falciparum, vivax, knowlesi (ไม่ใช่แค่ falciparum) — ถ้า met WHO severe criteria → รักษาแบบ severe ทันที.
แยกบนสเมียร์ (จำ):
- P. falciparum — ส่วนใหญ่เห็นแค่ ring form + gametocyte (banana/crescent), ไม่ขยายขนาด RBC
- P. vivax — RBC โตมาก + บิดเบี้ยว + Schuffner’s dots
- P. ovale — RBC โต + Schuffner’s dots + comet form / fimbriae
- P. malariae — RBC ขนาดปกติ + band form / rosette / compact form
- P. knowlesi — หน้าตาเหมือน malariae แต่ parasitemia สูงมาก
🩻 Rickettsial & Other Vector-borne Diseases
Rickettsia (ไทย 2 โรค):
- Scrub typhus (Orientia tsutsugamushi, ไทย 6 สายพันธ์, vector = chigger mite) — incubation 5–10 วัน, eschar (firm adherent black scab 3–6 mm ± erythema, พบ 50–80% ที่ axilla/scrotal), severe headache, ตาแดง; severe → meningoencephalitis, myocarditis, ARDS, AKI, septic shock
- Murine typhus (Rickettsia typhi, vector = rat flea) — incubation ยาวกว่า ~12 วัน, ไม่มี eschar (เข้าทางกิน/หายใจ), ไข้ปวดหัว myalgia ± rash
- Tick typhus (spotted fever group) = ไทยรายงานแค่ครั้งเดียว (เชียงใหม่ 40 ปี)
eschar ≠ escalation — ตอนสอบบรรยาย lesion อย่าตอบชื่อโรค.
Other vector-borne (รีวิวเพิ่ม):
- Dengue (Aedes) — ไข้สูง เบื่ออาหาร
- Chikungunya (Aedes, alphavirus) — ไข้ + ปวดข้อ
- Zika (Aedes) — ไข้ + ผื่น + ตาแดง; สำคัญ: fetal anomaly (คนท้อง)
- Typhoid fever (Salmonella) — ไข้ + ปวดท้อง, ท้องผูก 50% / ท้องเสีย; food-borne
- Leptospirosis (ริดูฝน, โรคฉี่หนู) — ไข้ + ปวดเมื่อย, 2 phase (febrile → immune phase wk 2: renal failure, ARDS)
💊 Management
Malaria treatment algorithm
Severe malaria (any species):
- IV artesunate 2.4 mg/kg push at 0, 12, 24 h → OD จนกินได้ (ต้อง push — half-life 20 นาที, drip → ระดับยาตก)
- Alternative: quinine 20 mg/kg loading drip 4 h → 10 mg/kg drip 2–4 h (ห้าม push → QT prolong → death)
Non-severe (ต้องแยก species):
- Falciparum: ศรีสะเกษ/อุบล (DHA-PIP resistant) → artesunate-pyronaridine + primaquine; จังหวัดอื่น → DHA-piperaquine + primaquine. Primaquine ใน falciparum = gametocidal, single dose
- Vivax / Ovale (มี hypnozoite): chloroquine (ไม่ดื้อ) + primaquine ยาว (radical cure)
- Malariae: chloroquine. Knowlesi: chloroquine หรือ DHA-PIP (guideline prefer DHA-PIP)
Primaquine + G6PD: ซักประวัติ/ตรวจ G6PD ก่อน (hemolysis). G6PD def → 0.75 mg/kg/week × 8 สัปดาห์; G6PD ปกติ → 0.5 mg/kg/day × 7 วัน หรือ 0.25 mg/kg/day × 14 วัน.
Rickettsia treatment
- Gold standard = doxycycline 100 mg q12h; alternative azithromycin (500 mg day 1 → 250 mg)
- Scrub typhus: prefer doxycycline; murine typhus: doxy หรือ azithromycin ได้ทั้งคู่ (Thai RCT: azithro 3 วัน = doxy 7 วัน)
- รักษาตาม clinical suspicion ได้เลย (ไม่รอ serology)
📚 Diagnostics & Pearls
Rickettsia investigation: เพาะเลี้ยงไม่ได้ (intracellular) → serology / PCR. Weil-Felix test = เลิกใช้ (sens 5–47%, spec 44%). Gold standard = Immunofluorescent Assay (IFA) (sens/spec ~96%): IgM ขึ้นก่อน, titer >1:400 → diagnose; มาเร็ว→ follow-up wk 2 (4-fold rise). พร้อมกุฎ 9 ตรวจได้: scrub typhus, murine typhus, tick typhus.
Case pearl: ปัชชี 58 ปี ทำสวนปาล์ม → ไข้ + headache + eschar ที่ไหล่ (เพราะปีนบนใบปาล์ม) + ceftriaxone ไม่ดีขึ้น → scrub typhus → doxycycline.
